Michael Toland's comment on the Jay Reatard thread made me think of this.

Pretty much every member of the Mentors - Sickie Wifebeater, etc.

I listened to Reatard's Watch Me Fall a little while ago - good stuff. I know he adopted his surname from his punk band the Reatards, but still.

Chuck Roast (Suburban Lawns)

When I was a kid, my fave name was Dinah Cancer, from 45 Grave, a band that also featured one Rob Graves.

Mr. Anus, and Mr. Horribly-Charred-Infant (Happy Flowers)

Poly Styrene (X-Ray Spex); I'm surprised New Wave never produced a 'Polly Esther.'

Darby Crash was originally Bobby Pyn. I think he made the right decision.

Ron Reyes had the name 'Chavo Pederast' foisted upon him by Black Flag after he left the band, so don't blame him.
